• Your birthdate and zip code are requested since those fields are used to verify your identify for the free access to the apps for online workouts and credits for local gyms & studios. Please enter your birthdate in the MM/DD/YYYY format. Your zip code is your 5-digit zip code at your primary home address.
• For the group name, this is your organization, employer, community or group that is sponsoring this program.
